New Patient Portal

Patient Guide for our Patient Portal

From 19 July 2021 we are launching our patient portal for our enrolled patients age 16 years and over. This service will be offered using a ConnectMed service, which we already use for online appointment bookings.  You will need to have your mobile phone number and your own individual email address.  You can also access more information on ConnectMed via:   https://www.connectmed.co.nz/

There are two levels of access to use the service:

Level 1

Visit our website https://www.hoonhaymc.co.nz/ and click on the icon to book an appointment or order a repeat script.  The first time you use it you will need to click ‘join’ and follow the instructions.  It will enable you to:

  • Book and cancel appointments online and see your booked appointments. This service is already available
  • Order repeat prescriptions online – level 1 does not give you a full list of your regular medications to choose from, you need to type in each medication you wish to request. This will be available from 19 July 2021 and will replace the existing system on our website

Level 2

This will give you more access to your health information.  You need to register  to use the patient portal service.  Call in and complete a registration form.  This needs to be done at our clinic rather than via email as we need to verify your identity via photo ID.   Once you have completed the form you will receive instructions on how to activate your patient portal account.

You will then be able to login via our website https://www.hoonhaymc.co.nz/   At this level you will have access to:

  • Book and cancel appointments online and see your booked appointments. This service is already available
  • Order repeat prescriptions online – you will be able to select from your list of regular medications, plus request other medications as needed.
  • See your lab test results, immunisations, recalls, allergies and regular medications

NB you will be able to see entries which are made in your health record from the switch-on date of 19 July 2021
